WIN/ LOSS STATEMENT PROCESS:

Club Member Must Complete a Win/Loss Statement Request.  All information must be completely fill out or it will not be processed.


Send the Completed Form back to the Player's Club by:

  • Mail

  • Fax

  • In Person


 

3. Once the completed form is received by the Player's Club, please allow up to ten business days to receive your statement.

  • You can get your Win/ Loss statement printed in person by visiting the Players Club station.
